Fees and Payments

FEES:
Initial Evaluation $500
Psychotherapy $350
Medication Management Visit $250

Fees for forensic consultation will be provided upon request.

Dr. Crandall accepts the following insurance plans:

• United Behavioral Health
• United Health Care including Oscar
• Blue Shield (specific plans only, please inquire for details)

Fee for Service:
If a patient decides not to use insurance, has insurance that Dr. Crandall does not accept, or does not have insurance, payment in full must be received at the time of the appointment.  Cash, checks, FSA cards or credit cards are accepted; for credit, there will be a 3% administrative fee.  For patients using other types of insurance, a superbill will be generated which the patient can then send in for reimbursement.
